OPT never received what to do?

Ok so my opt was approved after a long delay and it was mailed to me on September 1st on usps it says delivered but I haven't received it yet. I called uscis and they really have very slow dyslexic people on the phone. The first time I called the lady told me to fill out for I 912 for fee waiver but didn't gave me any information what other documents should I send. Again I called after a week and the person told me u need to fill out didn't receive card category on their website. Now I did that and it says you will hear from uscis by November 3. By that time my opt 90
Days grace period will expire. Did anybody had the same situation if yes then please tell me what did you do.

Talk to you dso they can call and help you out... Also.check in you nearest post office if it returned it will be there for some time... Another ask you neighbours if you may they dropped it at a wrong location... Good luck
