Travelling to India in opt

Hi,

I am travelling to India on oct25th and returning back to USA on dec 5th. My OPt expires on jan31st. I have the following questions in my mind.

1.Will it be a problem to leave at that time.
2.What are the documents required to carry during immigration process.
3.Will 1 month 20days time is enough to complete my opt extension process.

It will be helpful for me to take your suggestions. Please let me know.
