Forgot to get my OPT I20 signed from DSO within 6 months

Hello guys,

I need help, I forgot to get my OPT I20 signed from my University DSO within 6 months. I got to know it just now and before contacting my university tomorrow I wanted to know how will it affect me. Will it cause termination of my status?
