  • Lost OPT approval letter and notice [URGENT]

    Hi,

    I lost my approval notice(I-797 notice of action) and a notice comes with OPT card looks like this (https://www.dropbox.com/s/ti6on0djwe...otice.jpg?dl=0) I have to apply for extension for my OPT STEM extension and wanted to check how can I get these lost documents back. The notice says, use this tear-off portion to speed your application for an extension or replacement card. I don't know what this notice is called I've tried calling USCIS many time but they are not understanding what I'm asking for even my DSO don't have any idea. Please help guys.

    Your help is needed and will be appreciated.

    I have never seen anything like this and frankly I am not surprised no one knows what you are talking about. I wouldn't worry too much about it at this point. It will take a while for USCIS to send you whatever that document is.
    Just don't worry about it and apply for OPT STEM Extension using the regular process.

      Hi, Thanks for reply.

      I was getting mad because it says send your EAD card with this and I lost it. Can I know Have you ever applied for STEM extension and if so, did you send your card with the notice or not?

        Applied with which card and which notice?

        When I applied for my OPT STEM Extension, I sent a copy of my 12 month OPT Card, Visa copy, Passport copy, Diploma copy, Job verification letter with e-verify#, I-94 card copy, copy of all I-20s and drivers license copy.

          Did you send any document looks like this? the gray box is covered because of sensitive information. There were bar-codes below the boxes and did you send copy of approval notice?

              I never received any such notice, nor have I heard of anyone else receiving this. I am unsure if they just started doing this or something. I did not enclose copy of my 12 month opt approval notice.
              The documents I sent are mentioned above. I also attached pay stubs as well.
              Like I said, if you lost that notice, just apply without it. USCIS will take weeks to get you a replacement for that notice you got.

                Thanks @aichteeque. I am thinking of doing that only but I had fear of getting denial or rejection of OPT.

                  No need for approval notice or any document which came with your ead card.. while applying u just need the current ead card and other doc what aichteeque has mentioned
