Replacement EAD card

Hello guys,
I got my OPT EAD card and i have lost my OPT card , I have applied for a new replacement card (filled I-765 and sent all the documents)
and after 90 days i got a letter saying your opt card is denied.
Can any one help me ?
what is the process now to a get the card ?

Denied because while reapplyin a new opt card /I765, you need to file a new i20, the one which they initially gives is valid for only 90 days, if you are doing a change , every time you have to look for i20 status .

Its been 160 days that I didnt get my old EAD and I want to apply for replacement, should I get a new signature from University office?
