OPT Extension Application

Hello,


I am in the process of applying OPT Extension.
For the 12 months OPT, applied to Nebraska Service center and I lost my card. Replacement card application is to California service center since I moved to new place.

For OPT Extension application for 11 th question
Which USCIS office before applied for employment authorization?

I have to fill as Nebraska Service center or California Service center?
Please let me know.
