Place of last entry into the U.S. on OPT Form

Hello,

I am filling my I-765 form for opt. It says place of last entry into U.S. My immigration was done on Chicago so I first lended to Chicago, stamping done their then I took connecting flight to St. Louis. So, I would like to know what would be my last entry into the U.S. Chicago or St. Louis? I have not travelled outside U.S. by that time.

Also, my eligibility criteria is (c)(3)(B). Do I still need to fill question 17 and question 18 in the form?

Chicago is your last place of entry as that is where you entered the U.S. Check your I-94.
Read the instructions. 17 is only for STEM OPT applications. 18 is for those who are in H-4 status and are applying for the EAD. These questions don’t apply to you.
