Returning back to USA on OPT

I have a travel signature on OPT I20 signed on January 21st. I got my EAD card on February 25th, which is also the start date. Now, I have been in India from january and have been working for my US company under OPT (pay is received to my Indian bank account) where i was interning last year. Now to return back to USA what all documents should i carry with me. Also, I have heard that since I have been out of country more than 5 months I may have to get my visa restamped, is this true ?

You will need to contact your International Student Advisor or DSO immediately. First, your I-20 needed to have been signed within the last 180 days, and it looks like it has been more than six months. Second, you are not supposed to travel outside the US for more than 5 months. In order to re-enter, you will need a new I-20 and SEVIS record. It’s unlikely that you can get a new I-20 as you are no longer enrolled in school and have already entered into the OPT period.
