• I-765 Q#11 Re-submission

Hi guys i have a question regarding to question11 on I-765.
Since I was rejected the first time of my Application for Employment Authorization and received the I-797C(because I forgot to sign it, for God sake!), I think when I resubmit the I-765, I should choose ‘YES’ to question#11.
But then I’m asked to fill ‘Which USCIS Office’ and ‘Dates’. i have no idea what to write for the USCIS Office because I don’t even have an EAD card indicating which office to which I applied.
And for the Dates, is it the date on my previous I-765, which is March 24, or the date on the I-797C, which is March 26 for the received date and March 30 for the notice date. Which date should I write? I saw the online instruction for I-765.It says to write the start date and end date on EAD card, which I don’t have.
I guess I should write ‘Denied’ for the following ‘Results’ question and just leave the ‘Which USCIS Office’ and ‘Dates’ blank. Is it correct?

OR shall I still simply choose ’NO’ for question#11, pretending nothing happened?(Seems a bad idea)

Also, what else do I need to enclose in my resubmission package besides the green letter saying ‘When you resubmit your application, please enclose this letter with your re-submission’?

I called USCIS 800-375-5283, but they said they couldn't answer form questions... WTH

Can anyone can help me address this question? Many thanks in advance!

Hi John.
I found the following case that is relevant to ur issue. Please have a look into it..

You have to be honest with USCIS. U cant hide anything. Just say ur previous application got denied and as a proof send all the denial proof and other communication that is relevant. I am sure you will get it second time.
Good luck.

Hi johncheung,

Could you update us with what you finally filled in the form and what happened thereafter. I did the same mistake, and I could not find any information online to filling up section #11 after getting i-797C form (Notice for Action)

Thanks!

Hi Rao,

I got my EAD card a couple of months ago.
Here is what I filled for Q#11
Check YES
Which USCIS Office LIN-Nebraska (The first three letters of you Receipt Number represents to which office you applied. You can look it up on USCIS website. In my case just putting 'LIN' would be fine, I guess )
Dates 03/26/2015 (This is my received date)
Results Denied

Do remember to include ALL the document of your first application (You will need to let ISSS of your college issue a new I-20 if the old one expired. The old I-20, though expired, should enclose in your re-submission.)

Hope my answer will help. Don't panic. Good luck!
BTW, please write some pros to the new 24-months OPT extension. We must do something now!
