I had the same issue at the end of Fall 2014. My OPT was approved and GPA went below the required GPA to graduate. I spoke to the concerned person of the International office about this. What she said is as follows " This is a very strange case, we need a day or two so that we can discuss it with the chairman of the international office and we'll let you know then." Four days later i got a call from the international office of the University and what she said is as follows "You have to take a course to achieve the required GPA and at the same time you have to work part time i.e., 20 hrs/week. And we will update your sevis" I asked her whether i can put a hold on my OPT i.e., can i reschedule my opt start date in May 2015? here is what she said "No, since your OPT is approved you have to work at any cost, in your case part time, and you have to take a course as well". Fortunately, I got an online course that is counted towards my graduation degree. It depends on the university whether you can take an online course or not.

          You have 2 options:
          1. You can ask the adviser to help you immediately withdraw your OPT application (before it gets approved). This will allow you to retake the course and then apply for OPT again later. Once your OPT is approved, you can only do option #2.
          2. Let the OPT get approved, and register for the class you need to redo. Once OPT comes, you can work up to 20 hours a week while you are taking the class. You should work because if you don’t work, you will accrue unemployment time, and you shouldn’t have more than 90 days of unemployment.

See the ICE guidance regarding OPT (Section 6.9).

If you continue to do both. You won't be able to apply for stem extension

i was in same situation , but i did not apply for OPT knowing that this might happen to me , in worst case you may lose OPT , because when you apply for OPT , it is "assumed" that you will graduate which you will not in your case , you have few options
                    1) first of all to be on safe side , apply for OPT cancellation ASAP before it is approved , once it is approved then process is even more longer and headache to use remaining part of OPT
                    2) get another i20 so that you can be in status
                    3) ask your professor , if required escalade this issue to your program chair at your Uni saying that it is very critical to change grade to 3 , you must graduate in order to use your "approved" opt ( which is not approved yet , fortunately ) , note that you do not need "good" gpa to get opt , the only problem is graduation

Yes without completing ms . If you are using your opt. Then you are not eligible to apply for stem extension.
