OPT: I-765/I-94 Question

Hello All,

I am currently doing Masters on F1 Status and on my Final Semester. About to apply for Post completion OPT.

I need one clarification regarding my I94 number.

Question related to I-765, Application for Employment. In that i am suppose to fill:

1. I-94 number
2. Date of last entry to US

I have confusion in my I-94 number so with Date of Last entry.
Back Ground:
I came to US during July 2013 on H4 visa. During Sep 2013 I had applied for COS (Change of Status) to F1. eg: I-94 number 12345
My Change of status from H4 to F1 is approved on Feb 2014 and applicable from 6th Apr 2014 onward with I-94 number mentioned as 12345
Due to personal emergency I had traveled (March 2014) to India. I Traveled back to US on March with H4 visa. Got new H4 I-94 number, let say 67890
I am currently on F1 Status
My Question is:
Which I-94 number I have to mention ( 12345 or 67890)?
Which date I have to mention as Last entry to US?
Is I-94 number and Entry date should match?

67890 the date u retured date after emergency you can check online in the web site ) 3 yes all this info will be available on the website when you type your paaport number in the link
