    Incomplete OPT Application Packet

    Hey Everybody,

    I have sent my OPT application with the following Documents:

    1) I-765
    2) Graduation Receipt from College
    3) Check for $380
    4) Passport Copy: front and back of passport
    5) i-94 online receipt
    6) Two Photographs.

    My question: Am I missing something here? Some websites say I need to submit i20 copy and visa page copy. Some websites say not required.
    Please advice what I'm missing in my application packet and what I need to do in case I have sent the incomplete package?

        The only instructions that matter are the instructions given by USCIS: http://www.uscis.gov/sites/default/f...i-765instr.pdf. On page 2, it states that you must submit a Form I-20 that was endorsed by your DSO within the past 30 days. They need proof that your DSO has authorized the OPT. If you have not submitted the I-20, then USCIS will send a “Request for Evidence” requesting the I-20. Once you send it, they will approve it. If you haven’t received the I-20 signed by your DSO, AND if it’s not too late, you may need to withdraw your application and reapply. You need to speak with your DSO immediately if this is the case.

          The USCIS must receive the following documents within 30 days of the issuance of your I-20 with 17-month extension recommendation and the signature on your Form I-765:
           Original Form I-765
           Form G-1145 (E-Notification of Application/Petition Acceptance)
           Photocopy of your diploma (as long as it specifies your field of study). If not, you must include a photocopy of your official transcript. Note: Your transcript must confirm that your degree was conferred.
           Photocopy of EAD card(s)
           Photocopy of your employment letter
           Application fee of $380
          o This should be a check (from a U.S. bank) or a money order payable to the “Department of Homeland Security”.
          o Write the 11-digit number from your newest I-94 card on your check or money order.
          o Don’t forget to sign and date the check. (Please see attached example of a properly written check.)
           Photocopy of your current I-20 – page 1 and page 3 with 17-month extension recommendation
           Photocopies of pages 1 and 3 of all previous I-20's
           Photocopies of your current, valid passport (biographic and photo pages) and U.S. visa stamp page in your passport.
           Photocopy of the front and back of your most recent I-94 Card OR print-out of electronic I-94 (available at www.cbp.gov/i94).
           Two (2) identical natural color photographs of yourself, taken within 30 days of this application. Note: If your photos do not meet USCIS requirements exactly, your EAD card will not be issued.
          o Please see attached specifications for the photos.
          o In pencil, please write your name and I-94 number lightly on the back of each photo.

            Opt I 20 is missing ..... call USCIS asap and send ur opt I20

              I20 is imp .. If u did not take opt i20 u better to cancel u r application and then send it again ( for opt u r dso will request opt dates in u r sevis )

                    Visa copy is nt imp but opt i20 is must...your opt start date-end date is metioned in i20

                      Guys ... How long we have to wait for OPT EAC number or receipt .. I submitted my document on 1st October still I didn't get recipt mail !!! Please advise

                        I-20 is important but don't worry they will send an RFE for requesting the missing documents !

                          Dso recommended I-20 is very important..talk to your dso and fix the problem.

                            dont worry they will send you a letter if they need any other documents . Visa pages does nt requireed
