Best time is to apply before 90days from the date of expiration of initial cycle.

      As per USCIS
Eligibility requirements for 17-month extension of post-completion OPT?
-The student must have a bachelor’s, master’s, or doctorate degree included in the STEM Designated Degree Program List.
-The student must currently be in an approved post-completion OPT period based on a designated STEM degree.
-The student’s employer must be enrolled in E-Verify.
-The student must apply on time (i.e., before the current post-completion OPT expires).

      In additon
-Check with DSO on the timeline for filing 17-months extension. Earliest could be 120 days prior to end of current OPT period. Latest, before the current post-completion OPT expires.
-USCIS processing time is 3 months
-You will have to submit a request with your DSO for OPT STEM extension. DSO will mail STEM Extension I-20 to student.
-Complete the necessary paperwork and submit following to USCIS within 30 days or less from issuance of the OPT STEM Extension I-20.
Form I-765 with USCIS, Form I-20 endorsed by the DSO, a copy of the STEM degree, and the required application fee.
Form I-765 is being amended to require the student to indicate the degree and provide the employer’s E-Verify information.

Also the mailing address of the USCIS office depends on which state you're filing your extension from...I don't know exactly about that. Can be found on USCIS! Good luck!
