It has to be done through ur company for refund becoz they also get the money back. Try to get it done from them. Go to ur HR of the company or payroll for this process and they should be able to help you. If they don't do it, then on IRS website and look for SSN and medicare refund form.

Social Security and Medicare Tax Refund
I have another question- I am on OPT and my company deducted Social Security and Medicare tax in Error. Now I am looking for a refund but company is not responding.
Question is -
How can I get refund?
Can I file refund with tax filing for year 2013.or do I need to file tax and refund separately.
Company also deducted tax in January 2014. How can I get this year refund as well?
