Need your input on Filing for OPT

I am graduating in spring 2014 and I am preparing to apply for OPT. I wanted to make sure I do it right. Here is my scenario. I got my visa stamping for X university and transferred to University Y. I didnt well and I got dismissal from University Y. I managed to get an admission and transferred to another university Z before my sevis got updated. I am finally graduating after such a mess I created.

I wanted to make sure I file my OPT properly. What documents do I need to submit and also be prepared in case of RFE? If anyone has/had similar experience, could you please share them? Thanks

Why are you assuming RFE, it tells me you are up to something fishy.

Transferring universities is common. You will need to send copies of all your I-20s(all schools)

Your DSO should be first point of contact. They will have detailed OPT checklist on the website as well.
