  • USCIS OPT Form Expired, Is it okay to use?

    I am graduating this semester and have to apply for OPT. But when i try downloading the OPT form i see the form expires on 2-28-13. Can we still submit the form which has expired or do we need to wait for the uscis to update a new form.And what are the documents we need to submit when we apply for OPT.

    Check the USCIS website. It says that the new edition of the form is coming and until then any edition no later than 05/27/08 can be used. It's on the home page of the website.

      Regarding documents please contact ISO of yout school... Every school has there own process.. Anyways I am giving some info..

      1. Check or money order for $380 payable to Department of Homeland Security
      2. Two (2) Passport size photographs with your name and I-94 number printed in pencil on back
      3. Completed I-765 form
      Copy of pages 1 and 3 of your new STEM extension I-20 (Do not send the original! Make sure you sign your new I-20 before making a copy.)
      4. Copies of previous I-20s
      5. Copy of passport, visa and front/back of I-94 card
      6. Copy of transcripts
