I received my OPT in Three weeks and Never received a receipt I-797. Now when the time has come to apply for STEM extension I am unsure of what to fill in Item 11 field Dates on form I-765 (http://www.uscis.gov/files/form/i-765.pdf).Can any one please help.
-Should I simply mention the start date of my OPT?
