queries about I-765.please help

Hello everyone,

I had few questions regarding filling I-765 form. Did anyone come across question number 16," Go to the 'who may file form I-765' instructions and place letter of eligibility category".My question is what will be the eligibility code for H4 applicants and where should one mail them?
