traveling to india between h1b approval to opt last date and opt last date to 30 sept

Hi guys,

If my H1B is approved on 25th April and my OPT end date is 29th June.

1) Do I need to get one more Cap-Gap I20 from my DSO?
2) Can I travel to and from India from 25th April to 29th June?
3) Can I travel to and from India from 30th June to 30th September?
