RFE on F1 status

I came in to US in 2013 Aug. I completed my first Masters in May 2015. Due to Mistake of the DSO My OPT got Denied.
After that I joined another school (Second Masters) and initiated F1 reinstatement

After 5 months I got an RFE from USCIS and I have 5 questions to answer.




Please help me in answering 1,5 question.

For 1 I really don't have reason except I want to stay in USA
For 5 I have documents for last 11 months and Don't have it from start
