Changing Internship Service Provider at ITU, San Jose

I know that they allow one time change in one trimester. However, does anyone know how does it actually works? I mean, suppose if I worked for 1 week at unpaid internship for full time and then wanted to go for paid internship, so does ITU people ask for any letter report from the previous employer where I worked for 1 week ? Or, it's just filling up new form and submitting the request for new employer?

Also, do they cross check with the employer? Please share you experiences.

Not sure what ITU rules are change of employer in one week.Good idea to share this on ITU page as well for quicker response from other ITU students.
