H1B Admin Processing Refused but Passport with Embassy

Hi, My wife went for H1 stamping in India(Mumbai), she was told that the Visa is Approved after a day the case status changed from Admin Processing to Refused. Is this the final decision. please let me know about this. Do we have to lose our hopes on H1 now. Thanks in Advance.
