OPT extension to Day1 CPT

Hello,
My Stem OPT is expiring in Feb next year, i am planning to apply for day 1 CPT on spring. So, i am just wondering how i can do a transition from opt to cpt so i can keep working without having gap or leaving working. I would really appreciate if anyone of you help me the process to do. Like sevis transfer etc.
